Colorado Health Network (CHN) is a statewide 501(c)3 not for profit community based organization established to provide case management and supportive services, medical and oral health care, behavioral health services as well as prevention services to communities throughout the state of Colorado. CHN has the following four offices in Colorado (each office has a separate directory listing): Colorado Health Network - Denver; Northern Colorado Health Network; Southern Colorado Health Network;Western Colorado Health Network
