Service Description:
Housing Outreach Pharmaceuticals Education (HOPE). Homeless outreach for HIV-positive clients managed by Visiting Nurse Association. Lunch provided by Project Angel Heart. HOPE provides basic health care assessments and support administered by skilled professionals. HOPE helps clients take care of themselves and stay on track with medical appointments and prescribed medications including assistance with filling mediplanners. HOPE offers lockers to store personal belongings and medications and a place to relax, read and socialize.

Eligibility:
To qualify for the HOPE Program you must be homeless or in transitional housing. You must have proof of your HIV positive status.
