Service Description:
In furtherance of VNA's Mission and commitment to a continuum of health care in the home and community supporting optimal well-being, independence and dignity, VNA provides day treatment services for homeless HIV/AIDS clients at a central Denver facility including health care monitoring and counseling, and amenities such as showers, laundry and hot meals. The Program works with the Denver Health Medical Center Infectious disease Clinic Outreach Program, the Colorado AIDS Project (CAP), and Project Angel Heart.
Eligibility:
Homeless HIV positive individuals
